COVID19 VACCINE INFORMATION

You can now contact the Swartz Creek Area Senior Center to be included on a sign-up list to receive the COVID-19 vaccine (note: the vaccine will not be distributed at the center)!  This applies to those 65 years or older in Phase 1B.
Simply call the center between 9:00 a.m. – 12:00 p.m. or 12:30 p.m. - 4:30 p.m., Monday through Friday.  Our phone number is (810)635-4122.  When you call, we will be requesting your name, date of birth, and contact phone number.  Please do not email the information as we want to confirm it with you for accuracy and provide additional updates.
The contact information will be given to the Genesee County Health Department on a daily basis for vaccination scheduling as vaccine becomes available. The health department will call you directly to make your appointment, but please be patient and please answer your phone and check for voicemails regularly during the next few weeks.
Please do not call more than one agency or senior center.  The VAAA and all senior centers in Genesee County are working together with the health department to ensure that those who are requesting appointments will be scheduled.  Calling multiple agencies or leaving multiple messages will not speed up this process.
More than 150,000 people in Genesee County are eligible to receive the vaccine under phase 1B, but not enough vaccine is currently available.  The availability of the vaccines is extremely low but will improve over the next weeks and months.
You may not receive a call to schedule your appointment for several weeks, because the health department does not find out how many vaccines will actually be available until three to seven days before clinics are scheduled.
Please do not try to call either here or the health department to track your position on the list as the most vulnerable people may be taken off the list sooner and vaccine availability will change from week to week.
Thank you for your cooperation in this process!

The registration form, which you must fill out before receiving the vaccine, is located in this section, as well as the Michigan Vaccination Prioritization Guidance Article.  Hard copies of the registration form are available at the center.

THIS REGISTRATION FORM MUST BE RETURNED TO THE GENESEE COUNTY HEALTH DEPARTMENT!
